Health Office Update

Dear Parent/Guardian:

With fall fast approaching, the beginning of school is just around the corner.  The Health Office at Ray wanted to provide this information to you with ample time to prepare should your child require medication during the school day for the 2024-25 school year.

New York State Law requires that:
-Parents or guardians personally deliver all medication to the school health office. All Controlled Medications must be counted with the school nurse and quantity delivered must be signed off by both the adult and school nurse.
-All medications must remain properly labeled in the pharmacy container or in the original over the counter (OTC) containers.
-A new prescription/medication form signed by the medical provider is required at the beginning of every school year.
-The parent must sign the medication permission form.

If your child requires any medication at school, please see our attached medication form that will need to be completed by your child's medical provider and signed by a parent/guardian. Your physician may use their own form if desired. You may deliver the medication form to the health office when dropping off medication or the provider may fax directly to the health office at the fax number below.

If your student will be carrying his/her inhaler or epi pen, the order must state that the student may independently carry and use their inhaler or epi pen, in order for the child to do so. Please see the attached form titled Attestation for Independent Carry and Use. Students are not able to carry their medication if we do not have an order stating this. Middle school is an excellent time to start the discussion with your child’s medical provider about if your child should be deemed able to independently carry and use these emergency medications.

-Allergy Emergency Care Plan
-Seizure Care Plan
-Diabetes Care Plan
-Emergency Care Plan

If your child has any of the above medical diagnoses, please complete the appropriate form and return it to the health office. If your child has any other medical needs not listed above, please contact the health office for further assistance.

Nurses will be available for medication drop-off on school days during school hours. For your convenience, we will also be available during the hours of Meet Your Locker:

Friday, 8/23, 12:30-2:30 PM

Monday, 8/26, 8:30-11:00 AM

Tuesday, 8/27, 3:00-6:00 PM 

 If you are NOT planning for your child to visit their locker, but still need to drop off medication, please call or email the health office to make this arrangement. Our hours during the summer are limited, and the preferred method to contact the health office is via email at this time.

Prior to dropping off medication, please make sure you have the following all together at the time of drop off.

-Provider’s order
-Parent permission form
-Medication (in original unopened container)
